With this control you can create custom forms for your users such as contact us or a feedback forms. Forms may have as many input fields as you want. Form elements can be single select boxes, single select radio buttons, multi select checkboxes, textboxes and textareas. When forms are submitted by your vistors they will be emailed to you automatically.

Form Title
This is the title of your form.

Form Description
This is a short description of what your form is for.

Alternate To Email Address
An e-mail address should be set up in the configuration tool before your form submissions will work. This email address in the configuration tool is where your form will send an email to by default. Enter an alternate email address here if you want the form to send to a different email address.

Email Subject
This is the subject of your email.

Submit Confirmation Message
When visitors submit the form and it was successful they will be given this message.

Submit Failure Message
When visitors submit the form and it fails they will be given this message.

Submit Button Text
This is the submit button text for the form.

Reset Button Text
This is the reset button text for the form..

Use Challenge Response (humanity check)
Checking this box puts a CAPTCHA control on your form that forces visitors to enter in a sequence of characters before submitting a form, this helps deter bots (automated systems) from spamming your forms, this also has an adverse effect of making your forms less accessible.

Alternate Submit URL
After a visitor submits a form you can direct them to a URL entered in this field.

Text Entry (Single Line)
Enter your Question Text and an optional Description. The form will display a text box for your users entry.

Text Entry (Multiple Line)
Enter your Question Text and an optional Description. The form will display a text area for your users entry.

Single Select (Select Box)
Enter your Question Text and an optional Description, then add as many answers as you want for your question. The form will display a select box will all your possible answers for your users to select from.

Single Select (Radio Button)
Enter your Question Text and an optional Description, then add as many answers as you want for your question. The form will display a list of radio buttons of all your possible answers for your users to chose from.

Multi Select (Checkboxes)
Enter your Question Text and an optional Description, then add as many answers as you want for your question. The form will display a list of check boxes of all your possible answers for your users to chose from.

Reply to E-mail Address
This field is used to collect an email address from your visitors, the value in this field will be used when replying to your form submission.

File Upload
This field allows your visitors to upload a file attachment with their form submissions
